How should I take my antibiotics after abortion?

Everyone is prescribed antibiotics to help minimise the risk of getting an infection.

You will be given Metronidazole (2 tablets taken in the clinic) and Azithromycin (4 tablets to take at home). Sometimes we give you different antibiotics depending on which NHS area you are from, and whether or not you have allergies to the drugs we use.

The four Azithromycin tablets (1 gram each) should all be taken at the same time on the evening of your treatment. They may be taken either an hour before a meal or 2 hours after. However, if you're feeling sick or unwell after treatment, they may be left until the following day. It's advisable not to take the antibiotics within an hour of going to bed.

Here are some common questions about antibiotics, with answers.

  • What should I do if I've forgotten to take my antibiotics? Take them the next morning or within 24 hours.

  • I have been sick after taking the antibiotics; will I need to take them again? The antibiotics are prescribed as a precaution only. However, they also treat chlamydia. If you've tested postitive for chlamydia, you will need to see your GP as soon as possible for more antibiotics. You should also take care to follow the aftercare advice regarding how to minimise the risk of infection. 

  • Why have I been given antibiotics for longer? Which antibiotics you are given and for how long will depend on how your abortion has been funded. We have contracts with several agencies and they advise us which antibiotic treatment they want us to give to you.

  • Can I breastfeed whilst taking the antibiotics which were prescribed to me:

Azithromycin - We would recommend that you do not breastfeed for 48 hours after taking Azithromycin

Metrodazole - We would recommend that you avoid breastfeeding for 48 hours after taking Metrodazole 

You may choose to express some milk before attending the clinic to get you through until you can breastfeed again.

  • Can I drink alcohol while taking the antibiotics? You should not drink alcohol while taking your antibiotics or for 48 hours afterwards.


  • I have diarrhoea and stomach cramps after taking the antibiotics.  Diarrhoea is a common side effect of the Azithromycin antibiotics and may last a couple of days.

  • I'm in pain. The antibiotics will not relieve the pain that you may be having - you'll need to take painkillers for this. You can take painkillers and antibiotics at the same time.
